To request a performance by the U.S. Fleet Forces band, please fill out the below request. Note: submission of your request does not guarantee acceptance, but constitutes an official request for musical support. All band engagements are subject to preemption by Commander, U.S. Fleet Forces Command.

Important information about requests: 

• Requests will be reviewed to determine whether Department of Defense or Department of the Navy regulations permit band support for your event.

• Requests are processed on a first-come, first-served basis.

• Military events are confirmed no earlier than 90 days prior to the event.

• Civilian events are confirmed no earlier than 60 days prior to the event. 

How to Request a Performance

To schedule a performance by the U.S. Fleet Forces Band, please download and complete the appropriate request form as seen below. Request forms must be submitted by fax or email before your request can be considered for acceptance.
