NORFOLK, Va. - Capt. Matthew "Wrecking Ball" Wellman, Carrier Strike Group (CSG) 4's lead for training and assessments, discusses the Navy’s Root Cause Analysis Tool (RCAT) during the inaugural “RCAT University” at CSG-4 Headquarters, Feb. 27 to 28. The two-day course provided a platform for organizations across the Navy enterprise to have in-person and virtual discussions on improving fleet performance. RCAT began development in August 2022 to serve as a database to store and archive causal factor observations during military exercises and enable the principles of continuous improvement. Since its inception, RCAT has undergone over 65 iterations to best support exercise assessors who now use the program for various fleet-wide assessments.
